Pennsylvania Sen. John Fetterman is facing renewed criticism after a Wall Street Journal investigation detailed a series of incidents involving his approach to Senate duties, including a reported decision to dismiss a meeting request from Children’s Hospital of Philadelphia over Medicaid cuts and vaccine policy.

The Journal’s investigation, published Friday, September 4, is based on interviews with more than a dozen former staffers and lawmakers, along with text messages and other internal records. The report portrays a senator who has increasingly limited his involvement in routine congressional work while devoting significant attention to conservative media appearances and his strong support for Israel.

Fetterman’s office did not respond to multiple requests from the Journal for comment on the reporting.

The Children’s Hospital of Philadelphia Meeting

One of the most attention-grabbing episodes involves Children’s Hospital of Philadelphia, commonly known as CHOP.

According to the Journal, an aide told Fetterman last September that CHOP officials wanted to meet with him because they were concerned about Medicaid cuts and vaccine policy. Fetterman reportedly questioned why he needed to continue meeting with the hospital.

The Philadelphia Inquirer, which reviewed the Journal’s reporting, said Fetterman speculated to an aide that the hospital officials “just want free trips to DC.”

The episode has drawn particular attention because Medicaid funding is a major issue for hospitals that serve large numbers of low-income children. The Inquirer reported that roughly half of CHOP’s patients rely on Medicaid coverage and that the hospital had been warning federal lawmakers about the potential impact of Medicaid reductions.

Fetterman has previously taken a sharply negative position on the Republican-backed changes to Medicaid. During the debate over the legislation last year, he said he was voting “HELL NO” and described the bill as a disaster, according to the Inquirer.

Veterans Were Also Left Waiting

The Journal investigation also describes a June meeting involving three paralyzed military veterans from rural Pennsylvania.

The veterans had traveled to Fetterman’s office to discuss legislation affecting their care. According to people familiar with the incident, Fetterman canceled the meeting, telling staff that he was not feeling well.

The Journal reported that less than an hour later, Fetterman appeared at his office for an interview with Fox News focused on Israel and the war in Iran.

The contrast between the canceled meeting and the television appearance has become one of the most widely discussed elements of the investigation.

Questions About Senate Attendance

The Journal also examined Fetterman’s attendance at Senate committee hearings.

According to the report, Fetterman missed 11 of 12 full hearings of committees to which he was assigned during one period. Former staffers told the newspaper that aides later worked to make sure he was recorded as present before he left the room.

The Journal reported that since June of the previous year, Fetterman was marked present at 26 of 31 full hearings of his committees. However, transcripts showed he did not speak during 20 of those hearings.

Fetterman’s critics are using those figures to question whether he is adequately carrying out the responsibilities associated with his Senate seat.

Fetterman’s Relationship With Democrats Has Changed

The report also highlights Fetterman’s increasingly complicated relationship with his own party.

Fetterman was elected in 2022 as a populist Democrat with a strong working-class image. Since then, his political profile has shifted substantially, particularly over Israel and the war in Gaza.

He has become a regular presence on conservative media while clashing with progressive Democrats over his support for Israel. The Journal reported that Fetterman became increasingly isolated from Senate Democrats and skipped regular caucus meetings and lunches.

In March 2025, when aides told him that Senate Democratic leadership was organizing a special call, the Journal reported that Fetterman responded, “I truly don’t care.”

Fetterman has also fueled speculation about his long-term future within the Democratic Party. In July, he said he would consider leaving if Democrats became an “anti-Israel party.”

He has denied speculation that he plans to switch parties.

Democrats Are Now Turning Up the Pressure

The Journal investigation has triggered a new wave of criticism from Pennsylvania Democrats.

Rep. Brendan Boyle called Fetterman a disgrace, while Rep. Chris Deluzio criticized the senator over what he described as laziness, disrespect and his approach to foreign policy. Both lawmakers are viewed as potential figures in the political fight surrounding Fetterman’s Senate seat in 2028.

The political stakes are significant because Fetterman remains a sitting U.S. senator at a time when control of the Senate could be extremely close.

The Inquirer reported that Fetterman’s approval among Democratic voters has fallen sharply, while his standing among Republican voters has increased. That unusual political crossover has contributed to speculation about his future and the possibility of a competitive Democratic primary in Pennsylvania.
